INTRODUCTION

 Sokoine Regional Referral hospital it is under Tanzania government which provide its health care services as district hospital level also this hospital has high number of patients because the hospital area is near to Lindi Region town which consist high population  so according to the shortage of pharmaceutical personnel, shortage of essential medicine and shortage of other healthworkers come as a problem of this hospital

What was observed in that Lindi referral hospital has four dispensing departments which are; paediatric clinic which concerned children under five (5) years, Care treatment clinic (CTC) deals to those who have been affected from HIV/AIDS, Tuberculosis Clinic (TB) concerned to those infectious bacterial disease characterized by the growth of nodules (tubercles) in the tissue, National health insurance fund (NHIF) this department deals to those who have card of national health insurance fund,

The objective of being at Lindi hospital are to practice the all dispensing activities, Medical store keeping,pharmaceutical public health and monitoring and evalution of medicine use. Good dispensing ensure that, the right medicines dispensed to the right patients or clients and right route of administration which produce positive therapeutic effect to their patient.

CHAPTER TWO

DISPENSING UNIT

At  Sokoine Region Refferal hospital dispensing unit  is the depertment which  involve the dispensing medicine and medical supplies to patients who are either out  patient or in patients  and  temporally storage  of  medicine and  medical supplies  received from store . The dispensing unit  at  sokoine hospital  complises  actually four department  which are;

v Out patient dispensing department

v National healt insurance fund dispensing depertment

v Tuberculosis dispensing department

v Care and treatment clinic department

The objectives

 By the end of  practicum, I  was expected  to be able to:

 To establish clarity, completeness, authenticity  and legality of  presccriction

 To identify, pack and label medicine  and medical supplies  for dispensing

 Issue  medicines and medical supplies  to patients or clients  with clear instruction and advice

 Keep records  of dispensed  medicines and medical supplies

 Maintain a functional  and organized dispensing unit

 

OUT PATIENT DEPARTMENT

This is  the department  involve dispensing of medicine to exemption patient or client  who are under five  children and olders above 60 years old  who have no ability to pay money medication. And the  process of dispensing is operated by computerized system  so  as to manage the good documentation  and handling  medical records. The medicine in dispensing room arranged  in the shelves in a systematic manner, they are arranged according their therapeutic classes, according to expiry date.

NATIONAL HEALT INSURANCE FUND DISPENSING DEPERTMENT

National health insurance fund unit have two major parts which are

Shop medicine and medical supplies

NHIF medicine and medical supplies

NHIF medicines and medical supplies provided only to registered members where clients use card registered by NHIF after prepayment to seek health services, this unit supplied With many drugs dosage forms including parenteral ,orall,topical

Shop medicine and medical supplies

This involve provision of services where patients makes direct paying cost of treatments ,this system does not favour most of people due to high cost that poor economic people can not afford it

Skills or activities  that I have perfomed at NHIF dispensind department are as follows

v To identify  features of different parts of prescription

v To identify  prescription errors

v To relate  diagnosis  and prescribed medicines

v To give appropriate  instruction on the use of dispensed medicines  or medical supplies

v To check patients or clients understanding on use/precautions of dispensed medicines/medical supplies

v To enter information onof dispensed medicines and medical supplies in dispensing register oa prescription record book  

v To keep dispensing unit and its resources in good manner
